Does Cuba require proof of vaccination?

Update 2021-10-22: On Friday (22 October), the Federal Foreign Office points out that, as of 15 November, a vaccination card or certificate of vaccination in written form must be presented for the entry of persons over twelve years of age. PCR tests before and after entry are then no longer necessary.
