As of early 2026, there have been no recorded fatalities directly caused by a ride malfunction or accident on Goliath at Six Flags Over Georgia. However, there was a notable incident in 2006 shortly after the ride opened where a man suffered a fatal heart attack while on the ride. Medical investigations determined that the death was caused by a pre-existing heart condition that was triggered by the physical stress and excitement of the coaster, rather than a failure of the ride's safety systems.
