Despite its ominous name and "haunted" theme, there have been zero confirmed guest fatalities caused by the ride mechanics of The Twilight Zone Tower of Terror at any Disney park worldwide (Florida, California, Paris, or Tokyo). While the attraction simulates a 1939 elevator accident that "killed" five fictional guests, this is purely part of the scripted backstory and pre-show. In real life, the ride is one of Disney's most sophisticated engineering feats, using high-speed motors to pull the carriage down faster than gravity rather than letting it free-fall. There was a notable incident in 2005 where a 16-year-old girl suffered a stroke after riding the Florida version multiple times, but medical examiners determined she had a pre-existing condition and the ride itself did not malfunction. The only major mechanical "accident" occurred in 1998 when a cable snapped on one of the lift cars; however, the emergency braking system worked perfectly, and while seven guests were treated for minor neck and back pain, no one was killed or seriously injured.
